Unaffiliated Investigator Agreement
This form needs to be completed for all investigators who are not employees of Virginia Mason Medical Center or the Benaroya Research Institute at Virginia Mason (BRI), except for:
- Investigators affiliated with an institution holding a cooperative agreement with the BRI IRB (including UW, FHCRC, Group Health Cooperative & Swedish Medical Center)
- Research projects currently approved by an IRB holding a Federalwide Assurance (FWA). Contact your current IRB to see if they hold an FWA.
For the purposes of this form, an investigator is defined as someone who is involved in the research investigation itself (e.g. consenting, data collection and reporting), not solely in the provision of standard-of-care services or in the provision of a specific and limited study service (e.g. blood draws).
